- Payment information: payments are processed by our payment processor. We do not store full card numbers on this website.
2. Please don’t send health details through the website
Our contact form and chat are for general questions and scheduling. Please don’t include detailed medical information, test results, or medication history. Your provider will gather that information securely during your visit or through our patient portal.

Medical records created during your care are kept confidential in line with Ohio law and professional standards. They are separate from website data and are not covered by this website policy. Our patient portal is provided by MD-HQ and is also subject to MD-HQ’s terms and privacy practices.
